Jeff Horacek, MD, is an Institute for Functional Medicine Certified Practitioner (IFMCP). Dr Horacek has a particular passion for serving those with chronic illness. After 20 years of practicing traditional Internal Medicine Primary Care, he discovered the healing power of Functional Medicine.
