Jeff Horacek, MD, is an Institute for Functional Medicine Certified Practitioner (IFMCP). Dr Horacek has a particular passion for serving those with chronic illness. After 20 years of practicing traditional Internal Medicine Primary Care, he discovered the healing power of Functional Medicine. Personally and professionally, this way of treating disease makes complete sense to Dr Horacek.

In 2017 he founded Healthy Connections Hood River, along with his wife Heather, to serve the Columbia Gorge and Portland Metro communities. We also serve clients via our online courses and workshops from anywhere in the world!
